Augusta Health Center by Harborview is located directly beside Doctors Hospital providing seamless provision of services for our residents and a smooth transition to our center following a surgery or illness. Additionally, Augusta Health Center accepts referrals from Augusta Health University, Piedmont, and other hospitals and doctors offices across East Georgia and South Carolina.
In addition to 24-hour skilled nursing services, we offer onsite physician/nurse practitioner services 5 days per week, with after hours on-call services available 7 days per week. Our dedicated physical, occupational and speech rehabilitation staff are in-house and employees of the center. Augusta Health Center is a nursing agency free environment.
We offer a host of amenities including spacious resident rooms, free salon services at our Esteem Salon and a first class dining experience. We are also proud of our outdoor covered patio space and other scenic outdoor spaces.
Our Services
Augusta Health Center by Harborview uses individual care plans to help seniors with their long-term care, skilled nursing, and rehabilitation needs. It has specialized care tracks, including cardiac and respiratory care. We also provide:

DEMENTIA / ALZHEIMER’S CARE
Dementia is not one specific disease, but is a term used to describe a variety of symptoms caused by disorders that affect the brain. These disorders are characterized by impaired memory and difficulty with language, motor activity, recognizing objects and “executive function,” which refers to the ability to plan and organize.
Alzheimer’s Disease is a progressive brain disease that slowly, over time, destroys memory and thinking skills. It is the most common cause of dementia among older people.

At Augusta Health Center, your loved one will receive around-the-clock care in a quiet, comfortable setting and be engaged in a variety of activities designed to provide a sense of self-worth and purpose. We also use music or memory programs to help patients enjoy each day to its fullest.

How does the admission process work?
We will schedule a time to help you complete your admissions paperwork within the first 24 hours. You are welcome to bring any additional questions regarding your insurance benefits and your stay at this time or at any time while you are in the center.
We also offer an electronic admission paperwork option which allows e-signature via email if you have a busy schedule or are still at the hospital.
What should we bring to our admissions meeting?
- Active medical insurance card and Medicare / Medicaid cards
- Pharmacy (Part D) Prescription coverage card
- Driver’s license or photo ID card
- Social Security Card
- Advanced Directives Do Not Resucitate (DNR) paperwork
- Power of Attorney documents (medical, financial, durable)
- Guardianship paperwork
How do we establish a specific personal care plan?
Your physician communicates medication and treatment orders at the time of your hospital discharge. These orders will be implemented to help provide continuity of care.
Upon admission, the interdisciplinary team will work with you and your family to identify care needs, preferences, and goals for your stay. You are a valuable partner in the process.
Within the first few days of your stay, you and your family will be invited to participate in a care planning conference to discuss needs, discharge planning and progress toward goals.
